Home Health Aide

The Health Link Home Health Aide team provides the best daily care services in a patient’s home who either has an injury, a chronic illness or a disability. Under the supervision of a Registered Nurse or Therapist, our team of Aides create a safe environment for adults and seniors needing therapeutic and rehabilitative home health assistance for both long and short-term plans of care. Our mission is to enable patients to remain and recover in their own homes by tracking their condition and deliver support services. We educate your family members in patient care by teaching them the proper ways to turn, lift and reposition the patient and provide counseling on nutrition, housekeeping and cleanliness. We maintain a secure, safe and healthy living environment by maintaining security precautions and following procedures as well as prescribed dietary requirements and nutritional standards. Additionally, we keep daily journal records of a patient’s daily progress and notify the primary physician of unexpected or changing conditions. Furthermore, we observe physical and mental health, fluid intake and output and the achieved level of exercise.

What do Home Health Aides Do?

 
  • Provide routine personalized care under the supervision of a Registered Nurse

  • Monitor the patient’s health and maintain up-to-date records of the patient’s physical and mental condition

  • Assist the patient with personal care: toileting, bathing, dressing, grooming and eating

  • Assist in ambulation and exercise according to the plan of care

  • Help the patient in and out of bed, chairs, wheelchairs and cars

  • Provide routine basic care: assist in self-administration of medication and check and record vital signs
