Who Qualifies For Home Health Care?

 

  • You are homebound: confined to the home

  • Are currently under the care of a physician receiving services under a plan of    care established and reviewed by the physician

  • Are in need of skilled nursing care, physical therapy, occupational therapy  and/or speech therapy on an intermittent or continuing basis

  • Anyone who has a medical necessity, such as those recovering from a surgery, an illness, a recent hospitalization or are in need of regular at-home medical care or aid

  • If you are living independently or in an assisted living facility without needing long-term nursing care

  • Have a physician’s order and valid insurance

  • Are referred through nursing homes, hospitals, rehabilitation units, a primary  doctor, family and/or friends
